Home and Community Based Individualized Support Services

Daniel, who receives services in the Early Intervention Program, is working on a game that requires him to match shapes, label shapes, and take turns with others.

The Home and Community Based Individualized Support Services Program allows individuals with intellectual disabilities and their legal representatives to choose support services needed in order to live successfully at home and to progress toward future goals. Needed services and providers are chosen by the individual and guardian with assistance from the Ray Graham Association Service Support Planning Team. Individuals choose from a range of services including personal support, community learning centers (developmental training), supported employment, behavioral, physical, speech/communication and occupational therapy, nursing care, behavioral intervention/counseling and transportation assistance

“Our son Daniel has been involved in Early Intervention since he was 9 months old and I cannot believe that in a few weeks he will be turning 3 years old. Daniel has made miraculous progress and developed a special connection with his Developmental Therapist Sandra. He is excited when she arrives and even on tougher days she and Daniel have productive play therapy and because of that he is well on his way to becoming an incredible little boy that many will never know was diagnosed with Autism.

Grace M obtained an indoor therapy gym and platform swing using her Medicaid Waiver Home Based Funding. Grace is now more calm and ready to learn new skills. Grace’s family is proud of her recent accomplishments, including the ability to follow 2-3 step directions and more effectively communicate her wants and needs.
